Provide Ongoing Support as new issues arise
Assistance and Support for Long Distance Caregivers
Support and Instruction Navigating Care Transitions such as hospital discharges, rehabilitation stays, and moves to assisted living or skilled nursing facilities
Geriatric Care Management Services (Available in Northeast Ohio)
We provide an extensive assessment of the older person’s current situation, often providing an in-home assessment, to identify the medical, social, safety, financial and legal needs that must be addressed. Terry’s training as a nurse and attorney allows her to develop, with the older person and his/her support network, a personalized Care Plan that identifies the older person’s needs, the actions that should be taken to address those needs, and who will be taking responsibility for those actions.. Once in place, we can assist the Caregiver to monitor and adapt the Care Plan as the older person’s situation changes over time. Assessment of Medical, Social, Safety, Financial, and Legal Needs.
